• Same-week appointments are available.
• No referral is needed. You do not need a referral from a primary care provider or another clinician to get started.
• We welcome children, adolescents, and adults ages 8 and older.
• New-patient visits go on our shared calendar with the first available of Julia or Priya. Our office confirms your time and who you will see.
• Online requests need at least 2 hours’ notice.

Please complete the requested intake paperwork before your appointment. Accurate information helps us prepare for your visit. The forms ask about:

• Symptoms
• Medications
• Treatment history
• Medical conditions
• Pharmacy preferences

Payment to confirm. The $225 new patient evaluation is paid in full to confirm your appointment, and a card on file is needed to book. We currently accept debit and credit cards only.

Scheduling an evaluation does not guarantee that any specific medication, including a controlled medication, will be prescribed.

Our office is at 303 Williams Ave SW, Suite 1131, Huntsville, AL 35801. Suite 1131 is on the second floor.

Medication follow-ups are 15-minute visits, in the office or by video, at $125 self-pay. As a current patient, please contact our office to arrange follow-ups with your provider. We monitor progress and thoughtfully refine treatment as your needs change.

Please cancel or reschedule at least 48 hours before your appointment. A $50 fee applies to a late cancellation or a missed visit.

Arriving more than 15 minutes late counts as a missed visit.

Rates shown reflect a 25% same-day self-pay discount. Payment is due at the time of service; we accept debit and credit cards only.

Insurance participation is in progress. We are completing credentialing with these plans, which are not yet considered in-network. We will update this website as each network becomes available.

We do not accept Medicare or Medicaid. Out of network, we can provide a superbill for you to submit to your insurance company. Reimbursement is determined by your individual plan and is not guaranteed.
